CROWN POINT and FISHERS, Ind., August 10, 2026: HealthCall, LLC, an electronic health record and care coordination platform purpose-built for care delivery in Mobile Integrated Health and Community Paramedicine, and Primary Record Inc., a patient-directed health information platform for families and community care teams, today announced a formal partnership to make critical health information more accessible where care increasingly occurs: in patients’ homes and communities.

The companies are integrating HealthCall’s longitudinal record of MIH and community-based care with the health story patients and caregivers assemble in Primary Record. The partnership brings more complete, relevant information to patients, families and authorized care professionals across healthcare and community settings.

Closing Healthcare’s “Last-Mile” Information Gap

Health information is often fragmented across hospital systems, physician practices, patient portals, EMS records, printed documents and the memories of patients and caregivers. Although each organization may hold part of the record, no single provider necessarily has the complete picture.

That gap is especially challenging for MIH and community paramedicine teams caring for people with complex needs at home. Providers may arrive with an incomplete referral or limited visibility into care delivered by another health system, a previous EMS encounter or changes between visits.

Patients and families also struggle to manage multiple portals, medication lists and paperwork. Primary Record addresses that burden through consumer-directed exchange, giving patients direct, usable access to records from connected sources representing approximately 80% of U.S. hospital capacity. This model aligns with federal patient-access and information-sharing efforts led by HHS through ONC under the 21st Century Cures Act.

The partnership closes this “last-mile” gap by connecting Primary Record’s patient-directed record with HealthCall’s MIH encounter information and extending relevant information beyond institutional systems to patients, caregivers, and authorized professionals in homes and communities. Authorized providers gain greater context, while patients and caregivers retain greater control over how their information supports care.

More informed care in the home. Authorized MIH professionals can begin an encounter with greater context about the patient’s history, previous care and current needs, reducing reliance on incomplete referrals or recollection.

A more complete health story. MIH visits, home assessments and community-based care can become part of the record patients and caregivers use to coordinate care across organizations.

Patient-directed sharing. Patients and caregivers can authorize relevant information to be shared with the professional helping them at that moment.

Less friction and better continuity. Bringing information together can reduce time spent switching among portals, locating records and reconstructing medical histories. It also supports clearer handoffs among hospitals, clinics, EMS agencies, homes and community programs.

Greater visibility over time. Longitudinal information can help care teams recognize recurring events and changes, such as repeated falls, frequent 911 calls or shifts in clinical measures, that may be difficult to identify from one encounter. This is particularly valuable for older adults, people with multiple chronic conditions and families coordinating care among several providers.
