HealthCall and Primary Record Formalize Partnership to Connect
Mobile Integrated Health With the Patient’s Full Health Story

FOR IMMEDIATE RELEASE

New bidirectional integration connects on-scene MIH records with patient-directed health information from hospitals, clinics and other care settings

More informed care in the home. Authorized MIH professionals can begin an encounter with greater context about the patient’s history, previous care and current needs, reducing reliance on incomplete referrals or recollection.

A more complete health story. MIH visits, home assessments and community-based care can become part of the record patients and caregivers use to coordinate care across organizations.

Patient-directed sharing. Patients and caregivers can authorize relevant information to be shared with the professional helping them at that moment.

Less friction and better continuity. Bringing information together can reduce time spent switching among portals, locating records and reconstructing medical histories. It also supports clearer handoffs among hospitals, clinics, EMS agencies, homes and community programs.

Greater visibility over time. Longitudinal information can help care teams recognize recurring events and changes, such as repeated falls, frequent 911 calls or shifts in clinical measures, that may be difficult to identify from one encounter. This is particularly valuable for older adults, people with multiple chronic conditions and families coordinating care among several providers.

“MIH professionals often care for patients at the moment when complete context matters most, yet the information they need may be spread across several organizations and systems,” said Daniel Hayes, founder and chief executive officer of HealthCall. “By connecting HealthCall’s longitudinal care-delivery record with Primary Record’s patient-directed record, we can help authorized providers make more informed decisions in the home.”

“Families have become the connective tissue of a fragmented healthcare system,” said Jean Ross, RN, co-founder and chief executive officer of Primary Record. “This partnership gives patients and caregivers a practical way to bring their health story together and share the right information with the professional standing in front of them.”

The partnership reflects a shared view that interoperability must extend beyond data exchange among large healthcare institutions. Information must also reach patients, caregivers, paramedics, community health workers and other professionals responsible for care outside traditional clinical settings.

Together, HealthCall and Primary Record are building a new model for patient-centered interoperability; one designed to place relevant, actionable information in the hands of patients, caregivers and authorized care professionals when it can make the greatest difference.
